A loyalty program in which you prequalify for a bonus payment in addition to
the base payment for each case.

When your practice submits two or more aligner cases* per month, you automatically qualify for a TEAM BONUS paid monthly up to $1,000/month. 

• Submit 2-4 cases/month = $100 Visa GC Team Bonus 
• Submit 5-9 cases/month = $300 Visa GC Team Bonus
• Submit 10-15 cases/month = $600 Visa GC Team Bonus
• And the TOP Bonus level is 16 or more cases/month = $1,000 Visa GC Bonus

*Only aligner cases are considered for the program; and, all treatment packages are eligible. Once the treatment plan (“FXPlan”) is approved and the patient pays for the treatment, the Team earns the bonus for the case.

Note: We consider the payment date or the plan approval date, whichever comes last, as the date on which the bonus is earned. Any case which is discounted more than 20%, Try It Kits, retainer subscriptions and staff treatment do not qualify. 

The program starts February 1, 2021. We will give you notice when the program is ending. We anticipate offering the program at least through Q1’21 to give you a chance to participate. 

The monthly bonus is sent in the form of a Visa Gift Card directly to the office. The team can spend the gift card as they prefer. 

The Visa Gift Card is sent one week after the close of the month. 

We calculate the collective sum of all cases for the month, from all locations, for the team bonus value. 

You are automatically included in the OrthoFX Team Bonus Program when your practice submits at least 2 cases per month.

Cancelled cases do not qualify for the Team Bonus program.

The Team Bonus program is reset on the first day of each month. Therefore, the gift card amount directly corresponds to the number of cases submitted each month.  

We offer additional rewards for our top practices in two categories: larger practices and smaller practices (determined by the number of hours the hygiene staff work per month). In each category, we determine the top 5 performing practices based on their case submissions over the past 6 months. These top practices receive:

• One free case* (no charge to the patient) as a promotional/charitable opportunity
• Entry into a raffle to win a $1,000 Visa Gift Card

*Free cases are not eligible for doctor payouts or bonus programs. 

The raffle is held every 6 months for our top practices, and conducted over Zoom so participating practices can attend. It typically will be scheduled on a Friday at 12:00pm CT with lunch provided by the OrthoFX Regional Manager. One practice from each group (larger and smaller practices) will be randomly selected as the winner of a $1,000 Visa Gift Card.
